How do we reassure patients that a move towards larger practices is in their interests? These changes could be difficult for older people or more vulnerable people who may not engage if there is only one central place for chronic disease management.

Engaging and involving patients in changes at their practice requires careful planning and consideration.  Working at scale should result in a better service for patients but practices must be prepared to listen to any concerns and identify ways to address them where appropriate.

The Communications Team at the CCG can advise and support you to engage with your patients effectively. You should also make use of the relationships you have built with members of your Patient Participation Group and seek their advice for communicating with a wider group of patients.
